EGFR-TKIs

EGFR-TKIs are personalised targeted cancer medications that block EGFR signalling — a key driver of cell growth in certain tumours, especially non-small cell lung cancer (NSCLC). First- and second-generation TKIs paved the way for these therapies whereas third-generation agents like osimertinib have treated patients with resistance mutations such as T790M.

Programmed death ligand 1 (PD-L1) inhibitors

Some tumour cells express PD-L1 on their surface, which binds to PD-1 receptors on activated T-cells, delivering an ‘off’ signal that inhibits T-cell proliferation, cytokine production, and cytotoxic activity. PD-L1 inhibitors are antibodies that block this interaction and restore the immune system’s ability to recognise and attack tumour cells. Pembrolizumab is an example of a PD-1 inhibitor and durvalumab is a PD-L1 inhibitor.

Poly(ADP-ribose) polymerase (PARP) inhibitors

PARP inhibition prevents repair of single-strand DNA breaks that occur during cell replication. In cancer cells with defective homologous recombination repair (BRCA1 or BRCA2 mutations), DNA strand breaks accumulate, resulting in cell death. This process is known as synthetic lethality, where the combination of PARP inhibition and homologous recombination repair deficiency is lethal to cancer cells but not to healthy cells with intact DNA repair mechanisms.

Chimeric antigen receptor T-cell (CAR-T cell) therapies

CAR-T cell therapy has marked a major advance in the treatment of haematological cancers. By reprogramming a patient’s own T cells to attack cancer cells, CAR-T therapies, such as those targeting CD19, have led to responses in leukaemia and lymphoma. Today, research is focused on solid tumours, off-the-shelf (allogeneic) CAR-Ts and novel antigen targets.

Antibody–drug conjugates (ADCs)

ADCs are precision therapies that combine the targeting ability of antibodies with chemotherapy. ADCs deliver cytotoxic drugs directly to tumour cells, thus reducing systemic toxicity. Innovations in linker chemistry and payloads have expanded the ability of ADCs to target human epidermal growth factor receptor 2-positive disease as well as HER2-low and trophoblast cell surface antigen 2-expressing cancers.
